THIS EMPLOYEE BENEFIT PLAN is formally known as the Gates Hudson & Associates Health Plan (the “Plan”). The purpose of the Plan is to consolidate the multiple insured and/or self-insured health and
welfare benefit plans sponsored and maintained by the Employer into a single, comprehensive health and welfare plan, for ease of administration and reporting. This type of Plan is sometimes referred to as a “wrap” or “umbrella” plan.
