• STD Claim Form:  This document is your short-term disability form.  Please register online and follow the steps to start your STD claim.  The Group Policy ID is 171878-B and the Group name is Gates Hudson & Associates.  If approved, Short-Term disability will pay you weekly 60% of your gross salary up to $1,000.00 after a 15 day elimination period during the duration of your confinement as based on your physician’s recommendations.  During the 15 day elimination period, you are required to use Sick, Vacation, or Parental Leave (if eligible) to cover that time until the insurance kicks in and you start receiving your disability checks.
