The termination form should be used to document any incidents to include actions and/or behaviors resulting in terminating an employee's employment with Gates Hudson. Such examples include when an employee egregiously violates a Gates Hudson policy, standard operating procedure or process for which they have been trained and are fully knowledgeable of, or for any behavior which does not align with our Standards of Conduct.
The termination form may also be used in some cases where an employee is within his/her 90 introductory period (see Employee Handbook) and/or when an employee has received previous disciplinary action for similar or totally separate policy violations/violations to Standards of Conduct.
When drafting a termination, please include the objective facts of the case to include details of the following (the list below can also be found in guide form within the termination document):
- What happened? What was the adverse impact of the employee's actions and/or behavior?
- Dates/ Time of Day- When did the infraction take place?
- Were there any witnesses to the infraction and/or behavior?
- Any direct quotes which may be relevant to the reason for the disciplinary action.
- Include dates of any previous violations/behaviors for which the employee was disciplined.
- Include corrective actions/behaviors for the employee to adhere to for future evaluation.
